ABSTRACT

ABSTRACT Objective: To describe the first known case of human Bertiellosis in Paraná (Brazil). Case description: A 6-year-old male residing in the Brazilian state of Paraná was suffering from intermittent nonspecific abdominal pain and abdominal distension, associated with expulsion of live tapeworms in his feces for 7 months. He had a history of interaction with monkeys on an island. His first feces analysis was inconclusive, with an initial hypothesis of an atypical Taenia. However, after additional research, the parasitologist identified pregnant proglottids of Bertiella sp. The patient was initially treated with an unknown dosage of albendazole and nitazoxanide, as it was believed he had been infected with Taenia sp. Since the symptoms persisted, praziquantel 10 mg/kg was prescribed without further proglottids elimination. Comments: Human Bertiellosis is a rare zoonosis, with less than one hundred cases reported. However, it is a cause of chronic abdominal pain and should be kept as a differential diagnosis, especially in cases reporting recurrent tapeworm expulsion in feces and refractory treatment with albendazole.


RESUMO Objetivo: Descrever o primeiro caso conhecido de bertielose humana no Paraná, Brasil. Descrição do caso: Criança de seis anos do sexo masculino, residente no Paraná, Brasil, apresentava dor abdominal inespecífica intermitente e distensão abdominal, associadas à expulsão de helmintos vivos em suas fezes havia sete meses. Tinha um histórico de interação com macacos em uma ilha. Sua primeira análise de fezes foi inconclusiva, com hipótese inicial de uma Taenia atípica. No entanto, após pesquisas adicionais, o parasitologista identificou proglótides gravídicas de Bertiella sp. O paciente foi inicialmente tratado com uma dosagem desconhecida de albendazol e nitazoxanida, pois se acreditava que havia sido infectado por Taenia sp. Diante da persistência dos sintomas, foi prescrito praziquantel 10 mg/kg, sem mais eliminação de proglótides. Comentários: A bertielose humana é uma zoonose rara, com menos de cem casos relatados. No entanto, é uma causa de dor abdominal crônica e deve ser mantida como diagnóstico diferencial, principalmente nos casos que relatam expulsão recorrente de helmintos nas fezes e que são refratários ao tratamento com albendazol.

Introducción: La infección por Fasciola hepatica es una enfermedad zoonótica de distribución mundial, desatendida y subdiagnosticada. Objetivo: Determinar la frecuencia de Fasciola hepatica en una población preescolar en Tartar Chico, distrito de Baños del Inca, en la región Cajamarca. Métodos: Estudio transversal en 48 niños de una institución educativa inicial. Los padres entregaron 3 muestras de heces para el estudio parasitológico seriado y completaron una encuesta epidemiológica. La identificación de F. hepatica y otros parásitos se realizó con las pruebas de sedimentación rápida de Lumbreras, examen directo y Kato-Katz. Para describir usamos frecuencias y porcentajes, para el análisis bivariado aplicamos Chi-cuadrado o prueba exacta de Fisher. Resultados: La frecuencia de Fasciola hepatica fue 4,17%. Además, estimamos una proporción de 8,33% para Ascaris lumbricoides, 4,17% de Diphyllobothrium pacificum y 2,08% de uncinarias; así como parásitos contaminantes Entamoeba coli, Blastocystis hominis. Conclusión: Encontramos una frecuencia de 4,17% de fascioliasis entre preescolares de una comunidad altoandina del Perú.


Introduction: Fasciola hepatica infection is a globally distributed, neglected and underdiagnosed zoonotic disease. Objectives: To determine the frequency of Fasciola hepatica infection among a preschool population in Tartar Chico, Baños del Inca, Cajamarca. Methods: Cross-sectional study in 48 children of an initial educational institution. Parents delivered 3 stool samples for the serial parasitological study and completed an epidemiological survey. The identification of F. hepatica and other parasites was carried out with the Lumbreras rapid sedimentation tests, direct examination, and Kato-Katz. For descriptive analysis, frequency and percentages were used, for the bivariate analysis, Chi-square or Fisher's exact test was used. Results: The frequency of F. hepatica was 4,17%. In addition, a proportion of 8,33% of Ascaris lumbricoides, 4,17% of Diphyllobothrium pacificum and 2,08% of hookworms; as well as contaminating parasites Entamoeba coli, Blastocystis hominis. Conclusions: A frequency of 4,17% of fascioliasis was found among preschoolers from a high Andean community in Peru.

ABSTRACT

Tongxie Yaofang, also known as Baizhu Shaoyaosan, was first recorded in Danxi's Experiential Therapy (《丹溪心法》) by ZHU Danxi in the Yuan dynasty. It is composed of Atractylodis Macrocephalae Rhizoma, Paeoniae Radix Alba, Citri Reticulatae Pericarpium, and Saposhnikoviae Radix, and serves as the representative prescription for the treatment of painful diarrhea. It has the functions of tonifying the spleen, emolliating the liver, relieving pain, and checking diarrhea, and is mainly used in the treatment of gastrointestinal diseases such as irritable bowel syndrome (IBS) and ulcerative colitis (UC). In addition, it is effective in treating gastrointestinal disorders with mental and psychological abnormalities, as well as obstinate anorexia in children, depression syndrome, and respiratory diseases. Experimental research and clinical practice have shown that Tongxie Yaofang has multi-component, multi-pathway, and multi-target characteristics in the treatment of diseases. The mechanism of Tongxie Yaofang in treating diseases is mainly attributed to anti-inflammation, immune function regulation, intestinal hypersensitivity improvement, emotion regulation, etc. Monoterpene glycosides, flavonoids, chromones, lactones, and other components contained play an important therapeutic role. The research on the systems biology of Tongxie Yaofang, such as metabolomics, proteomics, and network pharmacology, provides a scientific basis for clarifying its mechanism of action and expanding its clinical application. However, there are still some problems to be solved, such as difficulty in combining diseases and syndromes and lack of in-depth systematic research. Through the retrieval and collation of relevant literature, this paper systematically reviewed the material basis, pharmacological effects, and systems biology research of Tongxie Yaofang, aiming to lay a foundation for in-depth research on its mechanism in treating diseases and rational application in clinical practice.

ABSTRACT

ABSTRACT Objectives: to assess Popular Health Education practices on intestinal parasites, carried out by telephone contact with men living in urban communities in Rio de Janeiro, Brazil, during the COVID-19 syndemic. Method: a quasi-experimental, quantitative and descriptive study, carried out with men aged 20 to 59 years. Pre-test was applied, and participants were divided into two groups: control and experimental. Popular Education in Health practices were carried out with an experimental group, and post-test was applied for both. Results: health education practices were significant in the experimental group, with a reduction in incorrect answers. There was an increase in incorrect answers in the control group's post-test. Conclusions: the Brazilian National Policy for Popular Education in Health contributed to qualify men's health literacy on intestinal parasites. Practice by telephone contact proved to be a powerful strategy for nursing and public policies to access this group and promote health in Primary Health Care.


RESUMEN Objetivos: evaluar las prácticas de Educación Popular en Salud sobre parásitos intestinales, realizadas por contacto telefónico con hombres residentes en comunidades urbanas de Río de Janeiro, Brasil, durante la sindemia de la COVID-19. Métodos: estudio cuasiexperimental, cuantitativo y descriptivo, realizado con hombres de 20 a 59 años. Se aplicó la preprueba y se dividió a los participantes en dos grupos: control y experimental. Se realizaron prácticas de Educación Popular en Salud con un grupo experimental y a ambos se les aplicó la posprueba. Resultados: las prácticas de educación en salud fueron significativas en el grupo experimental, con reducción de respuestas incorrectas. Hubo un aumento de respuestas incorrectas en la posprueba del grupo control. Conclusiones: la Política Nacional de Educación Popular en Salud contribuyó a cualificar la alfabetización en salud de los hombres sobre parásitos intestinales. La práctica por contacto telefónico demostró ser una poderosa estrategia de enfermería y de políticas públicas para acceder a ese grupo y promover la salud en la Atención Primaria de Salud.


RESUMO Objetivos: avaliar práticas de Educação Popular em Saúde sobre parasitoses intestinais, realizadas por contato telefônico com homens residentes em comunidades urbanas do Rio de Janeiro, Brasil, durante a sindemia de COVID-19. Métodos: estudo quase-experimental, quantitativo e descritivo, realizado com homens de 20 a 59 anos. Foi aplicado o pré-teste, e os participantes, distribuídos em dois grupos: controle e experimental. Foram realizadas práticas de Educação Popular em Saúde com grupo experimental e aplicado o pós-teste para ambos. Resultados: práticas de educação em saúde foram significativas no grupo experimental, com redução das respostas incorretas. Houve aumento de respostas incorretas no pós-teste do grupo controle. Conclusões: a Política Nacional de Educação Popular em Saúde contribuiu para qualificar a literacia em saúde de homens sobre parasitoses intestinais. A prática por contato telefônico mostrou-se uma potente estratégia para a enfermagem e as políticas públicas acessarem este grupo e promoverem saúde na Atenção Primária à Saúde.

ABSTRACT

Blastocystis spp. es el protista intracelular que en los últimos años ha infectado a más de mil millones de personas a nivel mundial. Sin embargo, el aumento en la prevalencia en México y su potencial patógeno son inciertos, por lo que este microorganismo aún se encuentra bajo investigación. Principalmente a nivel pediátrico, la blastocistosis es estudiada con mayor atención, debido a que destaca sobre otros agentes en diversos estudios realizados mundialmente y en México. El objetivo de este trabajo fue mediante revisión bibliográfica, evidenciar la frecuencia y su transición parasitaria como el agente más prevalente en la actualidad, pese a que no se asocia a sintomatología clínica.


Blastocystis spp. is the intracellular protist that in recent years has infected more than a billion people worldwide. However, the increase in prevalence in Mexico and its pathogenic potential are uncertain, so this microorganism is still under investigation. Mainly at the pediatric level, blastocystosis is studied with greater attention, due to the fact that it stands out over other agents in various studies carried out worldwide and in Mexico. The objective of this work was to show the frequency and its parasitic transition as the most prevalent agent today through a bibliographic review, despite the fact that it is not associated with clinical symptoms.

RESUMEN Introducción: las enfermedades inflamatorias intestinales constituyen un grupo heterogéneo de patologías inmunológicas crónicas, de curso no predecible, que incluye a la enfermedad de Crohn, la colitis ulcerosa y la colitis indeterminada, donde la susceptibilidad a la enfermedad, fenotipo y respuesta al tratamiento está determinada por la compleja interacción entre factores genéticos y ambientales. Objetivo: determinar los aspectos clínicos, epidemiológicos, endoscópicos e histológicos de la colitis ulcerosa en la edad pediátrica, en Pinar del Río en el período de enero de 2018 a diciembre 2019. Métodos: se realizó un estudio observacional, descriptivo y transversal en un universo y muestra constituido por 41 niños diagnosticados con la enfermedad en este periodo de tiempo. Para el análisis de los resultados se utilizaron los métodos de la estadística descriptiva como las frecuencias absolutas y relativas. Resultados: predominó el sexo masculino con una edad de entre uno y cinco años, como factores de riesgo predominaron los antecedentes de infecciones anteriores, la dieta inadecuada y el tiempo de lactancia materna menor de cuatro meses. Las manifestaciones clínicas más frecuente fueron: diarreas, rectorragia y el dolor abdominal. Conclusiones: se presentó la enfermedad con mayor frecuencia en menores de cinco años, de raza blanca y procedencia urbana. Las manifestaciones digestivas que predominaron fueron las diarreas con sangre, la rectorragia y las extradigestivas la pérdida de peso. La presencia de eritema, perdida del patrón vascular y las presencia de ulceras fueron los hallazgos endoscópicos más frecuentes encontrados. En el 85,5 % de los casos se presenta la colitis ulcerosa.


ABSTRACT Introduction: inflammatory bowel diseases constitute a heterogeneous group of chronic immunologic pathologies, of unpredictable course, including Crohn's disease, ulcerative colitis and indeterminate colitis, where disease susceptibility, phenotype and response to treatment is determined by the complex interaction between genetic and environmental factors. Objective: to determine the clinical, epidemiological, endoscopic and histological aspects of ulcerative colitis in the pediatric age in Pinar del Río in the period from January 2018 to December 2019. Methods: an observational, descriptive and transversal study was carried out in a universe and sample constituted by 41 children diagnosed with the disease in this period of time. For the analysis of the results, descriptive statistical methods such as absolute and relative frequencies were used. Results: the predominant sex was male with an age between 1 and 5 years, and the predominant risk factors were a history of previous infections, inadequate diet and breastfeeding for less than 4 months. The most frequent clinical manifestations were: diarrhea, rectorrhagia and abdominal pain. Conclusions: the disease occurred more frequently in children under 5 years of age, of white race and urban origin. The predominant digestive manifestations were bloody diarrhea, rectorrhagia and extra-digestive manifestations were weight loss. The presence of erythema, loss of vascular pattern and the presence of ulcers were the most frequent endoscopic findings. Ulcerative colitis was present in 85.5 % of the cases.

ABSTRACT Introduction: Tufting enteropathy is a rare cause of congenital diarrhea in neonates. It is characterized by the abnormal distribution of epithelial adhesion molecules, which causes enterocytes to shed into the lumen, forming the characteristic tufts. Case summary: A 15-day-old female neonate was taken by her parents to the emergency department of a tertiary care hospital due to diarrheal stools she had been experiencing since birth. The patient presented with dehydration, abnormal weight loss, metabolic acidosis, and acute kidney failure. She received treatment with alizapride, loperamide, zinc sulfate, and probiotics, but after 75 days of treatment she was still symptomatic. An upper tract endoscopy and colonoscopy were performed, finding flattening of the villi and lymphoid cells in the lamina propria. However, the symptoms persisted, and she died at the age of ten months. A post-mortem exome sequencing reported tufting enteropathy. Conclusions. When congenital diarrhea is present, tufting enteropathy should be considered. An early molecular study would allow to evaluate the possibility of performing an intestinal transplant or modifying the treatment to meet the patient's palliative care needs.


RESUMEN Introducción. La enteropatía en penacho es una causa rara de diarrea congénita en neonatos; esta se caracteriza por una alteración de la adhesión epitelial que ocasiona desprendimiento de enterocitos hacia el lumen y, en consecuencia, forma los característicos penachos. Se describe el caso de una paciente con esta patología. Presentación del caso. Neonata de 15 días de vida, quien fue llevada por sus padres al servicio de urgencias de un hospital de tercer nivel debido a que desde su nacimiento tuvo deposiciones diarreicas y a causa de esto presentó deshidratación, pérdida de peso, acidosis metabólica e insuficiencia renal aguda. La paciente recibió manejo con alizaprida, loperamida, sulfato de zinc y probióticos, pero a los 75 días de tratamiento continuaba sintomática. Se le practicó una endoscopia de vías digestivas y una colonoscopia que mostraron aplanamiento de las vellosidades e infiltrado de células linfoides en la lámina propia. Los síntomas continuaron y la menor falleció a los 10 meses de nacida. El resultado del exoma post mortem reportó enteropatía en penacho. Conclusiones. Ante la presencia de diarrea congénita, se debe sospechar de una enteropatía en penacho y considerar el estudio molecular temprano, pues este permite evaluar la posibilidad de realizar un trasplante intestinal o modificar el tratamiento según las necesidades de cuidado paliativo del paciente.

ABSTRACT

Adhesive intestinal obstruction is the most common type of ileus, and conserva-tive treatment serves as its preferred treatment option. In the course of conservative treatment, gastrointestinal decompression will relieve symptoms, prevent ileus progression and promote gas-trointestinal function recovery, which has significant clinical effects. Currently, decompression effects of nasointestinal tubes and nasogastric tubes are controversial. There is a previous Meta-analysis evaluating decompression effects of these two methods, but this analysis includes non-randomized controlled trial and lacks research about Chinese patients. Therefore, the authors con-duct a Meta-analysis to evaluate decompression effects of nasointestinal tubes versus nasogastric tubes for adhesive intestinal obstruction.

Introducción: La enfermedad inflamatoria intestinal es un padecimiento crónico de causa multifactorial, en los últimos años se ha incrementado el número de casos diagnosticados, por lo que su enfoque clínico-endoscópico e histopatológico es de vital importancia para proveer atención integral a los pacientes. Objetivo: Caracterizar clínica-endoscópica e histopatológicamente la enfermedad inflamatoria intestinal. Métodos: Se realizó un estudio observacional transversal, el universo estuvo constituido por 78 pacientes diagnosticados con enfermedad inflamatoria intestinal y la muestra quedó conformada por 54 pacientes. La información fue recogida en formulario a propósito de la investigación. Resultados: Predominaron los pacientes con edad de 50 años o más. Primó el sexo femenino y el color de la piel blanca. El síntoma cardinal más frecuente fue la diarrea y el tiempo de evolución que primó fue de cuatro a seis meses. La localización más frecuente de las lesiones fue el colon izquierdo y primó la colitis ulcerativa como principal variante. Predominó la mucosa de aspecto granular y las lesiones continuas en los rasgos endoscópicos, así como la distorsión de criptas y la inflamación de la lámina propia dentro de la histología. Sobresalió como principal complicación la fístula y de estos casos la mayoría requirió tratamiento quirúrgico. Conclusiones: Mayor afectación a pacientes de mediana edad, sexo femenino y de raza blanca. Principal síntoma asociado es la diarrea. Variante con mayor superioridad es la colitis ulcerativa. Adecuada correlación clínico-endoscópica e histopatológica. Las principales complicaciones encontradas fueron los trayectos fistulosos y la estenosis intestinal que requirieron en su mayoría tratamiento quirúrgico.


Introduction: Inflammatory bowel disease is a chronic disease of multifactorial etiology that in recent years has increased the number of diagnosed cases, so the clinical-endoscopic and histopathological approach is of vital importance to provide comprehensive care to patients. Objective: To characterize the inflammatory bowel disease clinical-endoscopic and histopathologically. Methods: A cross-sectional observational study was carried out. The universe was composed of 78 patients diagnosed with inflammatory bowel disease and the sample was made up of 54 patients. The data obtained from biopsy ballots were collected in a form. Results: Patients aged between 50 and 69 years old predominated. Female sex and white skin color prevailed. The cardinal symptom that predominated was diarrhea followed by enterorrhagia and the time of evolution that prevailed was 4 to 6 months. The most frequent location of the lesions was the left colon and ulcerative colitis predominated as the main variant. Continuous lesions and granular appearance of the mucosa predominated in endoscopic features, while in histological features crypt distortion and inflammatory infiltrate of the same layer prevailed. Among the complications, the fistula stood out and most of these patients required surgical treatment. Conclusions: Greater affectation in middle ages, female and white skin color patients. The main associated symptom is diarrhea. The most common variant was ulcerative colitis. Adequate clinical-endoscopic and histopathological correlation was evidenced. The main complication was the fistulous tracts and intestinal stenosis, that mostly require surgical treatment.

ABSTRACT Intestinal parasitic infections (IPIs) are neglected diseases caused by helminths and protozoa, with the relationships between parasite, host and environment having the potential to produce high morbidity and incapacity to work and mortality in vulnerable areas. This study assessed the prevalence of IPIs concerning socio-environmental conditions and analyzed the knowledge, attitudes and practices related to these diseases among men living in the slums of Rio de Janeiro city, Rio de Janeiro State, Brazil. A cross-sectional study was conducted in an agglomeration of urban slums between 2018 and 2019, with men aged between 20 and 59 years. A socioeconomic status questionnaire and an IPIs knowledge, attitudes and practices questionnaire (KAPQ) were applied. Coproparasitological diagnoses (n=454) were performed using four methods and samples of water for household consumption (n=392) were subjected to microbiological and physicochemical analysis. A total of 624 participants were enrolled. About 40% of the households had "water unsuitable for consumption". Only one Major Area, MA 3 was not statistically significant for IPIs (AOR=0.75; 95% CI: 0.30-1.88; p=0.55). The overall prevalence of IPIs was 23.8%. Endolimax nana (n=65, 14.3%) and hookworm (n=8, 1.7%) were the most frequently identified parasites. The analysis of the frequency of responses to the KAPQ has shown that men reported to seeking medical care if they were suspicious of IPIs, and around 35% would self-medicate. The results have shown the need to adopt integrated health education practices targeting male residents in urban slums to qualify the care with water for human consumption and promote self-care about IPIs. The household can be considered strategic for Primary Health Care activities for men.

ABSTRACT

Objective:To evaluate the safety and effectiveness of capsule endoscopy for the diagnosis of intestinal diseases in children.Methods:Clinical data of 113 pediatric patients who received capsule endoscopy in Xi'an Children's Hospital from October 2018 to September 2020 were retrospectively analyzed. The completion rate, passage time of stomach and small intestine, lesion detection rate, adverse reactions and complications of capsule endoscopy were analyzed.Results:Among 113 pediatric patients, 78 (69.03%) were male and 35 (30.97%) were female. The age was (99.8±44.7) months (9-195 months), and 31 (27.43%) were under 7 years old. The minimum weight was 9 kg and the minimum height was 70 cm. Eighty-seven pediatric patients (76.99%) swallowed capsules orally (the oral group) with the minimum age of 4 years and 3 months. Capsules were implanted in 26 pediatric patients (23.01%) under gastroscopy (the gastroscopic group), with the maximum age of 9 years and 2 months. Unexplained abdominal pain (47.79%) and unexplained gastrointestinal bleeding (31.89%) were common in the pediatric patients. The completion rate of capsule endoscopy was 97.35% (110/113), and the detection rate of lesions in small intestine was 31.81% (35/110). The passage time of small intestine in the gastroscopic group was significantly longer than that of the oral group (461.04±129.27 min VS 288.23±107.84 min, t=5.646, P<0.01). There was no significant difference in the passage time of stomach or small intestine among different genders, different ages or different endoscopic examination results ( P>0.05). The positive results of capsule were not correlated with the method of ingestion ( P=0.401, OR=2.562, 95% CI:0.284-23.077), gender ( P=0.154, OR=2.352, 95% CI:0.726-7.616), age ( P=0.949, OR=1.007, 95% CI:0.816-1.242), examination reason ( P=0.246) or small intestine passage time ( P=0.219, OR=1.003, 95% CI:0.998-1.008). No complications such as capsule retention occurred in any pediatric patient. Conclusion:Capsule endoscopy in children is noninvasive, rapid and simple, which can improve the diagnostic rate of small intestinal diseases in children, and can be further promoted in pediatric patients.

Abstract | Introduction: Intestinal apicomplexa protozoa are a recognized cause of gastroenteritis. They are endemic in Honduras and their epidemiology varies in different population groups. Objective: To identify risk factors for cyclosporiasis, cryptosporidiosis, and cystoisosporiasis. Materials and methods: We conducted a case-control study in a hospital-based population. We performed the diagnosis using the modifed Ziehl-Neelsen staining technique and collected the information from laboratory records and clinical charts. Results: Cyclosporiasis was associated with diarrhea (OR=2.28; 95%CI: 1.10-4.89), weight loss (OR=12.7; 95%CI: 2.49-122.00), watery stools (OR=2.42; 95%CI: 1.26-4.65), and infection with another protozoan (OR=3.13; 95%CI: 1.66-5.95). Cryptosporidiosis was associated with HIV infection (OR=15.43; 95%CI: 3.34-71.22), diarrhea (OR=3.52; 95%CI: 1.40-9.40), lymphopenia (OR=6.16; 95%CI: 1.99-18.98), and green color stools (OR=3.00; 95%CI: 1.23-7.30). Cystoisosporiasis was associated with HIV infection (OR=11.20; 95%CI: 3.53-35.44), diarrhea (OR=7.30; 95%CI: 1.89-28.52), leukopenia (OR=4.28; 95%CI: 1.33-13.75), green color stools (OR=11.59; 95%CI: 1.16-558.60), and Charcot-Leyden crystals (OR=11.59; 95%CI: 1.16-558.60). Conclusions: In this hospital-based population from Honduras, HIV infection was a risk factor for cryptosporidiosis and cystoisosporiasis, but not for cyclosporiasis.


Resumen | Introducción. Los protozoos Apicomplexa intestinales son causa reconocida de gastroenteritis. Estas parasitosis son endémicas en Honduras y su epidemiologia varia según los grupos poblacionales. Objetivo. Identifcar los factores de riesgo para ciclosporiasis, criptosporidiosis y cistoisosporiasis. Materiales y métodos. Se hizo un estudio de casos y controles en población hospitalaria. El diagnóstico se hizo utilizando la coloración modifcada de Ziehl-Neelsen. La información se obtuvo del registro de laboratorio y las historias clínicas. Resultados. La ciclosporiasis se asoció con diarrea (OR=2,28; IC95% 1,10-4.89), pérdida de peso (OR=12,7; IC95% 2,49-122), heces líquidas (OR=2,42; IC95% 1,26-4,65), infección con otros protozoos (OR=3,13; IC95% 1,66-5,95). La criptosporidiosis se asoció con el HIV (OR=15,43; IC95% 3,34-71,22), la diarrea (OR=3,52; IC95% 1,40-9,40), la linfopenia (OR=6,16; IC 95% 1,99-18,98), las heces de color verde (OR=3,00; IC95% 1,23-7,30). En la actualidad las investigaciones entorno a la calidad de vida desde la salud, son de vital importancia a nivel mundial, puesto que la identificación de los factores que desencadenan ciertas patologías prevalentes sirven para actuar y mejorar las condiciones de vida; en específico en este trabajo nos referiremos al grupo infantil y la parasitosis pediátrica, la misma que genera índices de anemia, peso bajo, entre otros. El propósito fue identificar la prevalencia de parásitos intestinales en escolares de 6 a 12 años en poblaciones rurales, de la ciudad de Paute, Ecuador. Se realizó una investigación de tipo cuantitativa, descriptiva y transversal, se trabajó con 608 niños pertenecientes a zonas rurales de Paute, el análisis coprológico fue mediante el método convencional, se reportó número de parásitos por campo. Esta investigación fue desarrollada bajo el permiso de un Comité de Bioética. Se analizaron 608 muestras coproparasitarias, obteniéndose una significancia estadística con respecto al sexo, los varones presentaron una prevalencia de parasitismo (14,63%) sobre las mujeres (8,88%), el parásito con mayor prevalencia es la Entamoeba histolytica. La prevalencia de parasitosis infantil en el sector rural de Paute, Ecuador es de 23,52%, siendo mayor en varones, el parásito más frecuente es la E. histolytica(AU)


Currently, the research about life's quality from a health's point of view has received tremendous attention around the world, because the identification of the facts that start a variety of prevalent pathologies are being used to improve life conditions. In this work we try the child group and its pediatric parasitosis, that produce anemia, low weight, and others. The purpose was to recognize the prevalence of intestinal parasites in scholar children from 6 to 12 years old in marginal populations of the city Paute, Ecuador. A quantity type of investigation was made, descriptive and transversal, using a population of 608 children from the rural population of Paute, the coprological analysis was using conventional methods, number of parasites were reported. This research was developed under the permission of the Bioethics Committee. 608 coproparasites samples were analyzed, getting a significative statistic in reference to sex. Boy's samples show a prevalence of parasitism (14,63%) in girls' samples (8,88%), the more prevalent parasite is Entamoeba Histolytica. The prevalence of children's parasitosis in the rural sector of Paute, Ecuador is 23,52% being more affected boys than girls. The most frequent parasite is E. Histolytica(AU)

Abstract | Introduction: HIV infection is still a public health problem worldwide and co-infections with other infectious agents including intestinal parasites are of particular concern, mainly in developing countries like Colombia. Objective: To conduct a cross-sectional study in patients attending an HIV care program in Antioquia given that there have been few intestinal parasites prevalence studies among the HIV population in the country. Material and methods: We evaluated stool samples from 192 patients by direct wet mount and concentration, modified Ziehl Neelsen staining, and agar plate culture. Univariate and correlation analyses were done to explore the association between socio-demographic and clinical characteristics and parasitological data. Results: The overall prevalence of intestinal parasites in HIV-positive subjects was 29.2% (56/192; 95% CI: 22.8% - 35.6%). Entamoeba histolytica/dispar/moshkosvkii with 13.0% (25/192; 95% CI: 8.2% - 17.8%) and Blastocystis with 12.0% (23/192; 95% CI: 7.4% -16.6%) were the most frequent. Opportunistic parasites like Cryptosporidium spp. and Cystoisospora belli were less prevalent, each one with 0.5% of positive samples (1/192; 95% CI: 0.1% - 1.5%). Commensal protozoa were also detected with a prevalence of 18.8% (36/192; 95% CI: 13.3% - 24.3%). Most of the individuals in the study had a controlled viral load and an LTCD4 count greater than 200 cel/µL. A small percentage (9.3%) had diarrhea. Bivariate analysis and multivariate logistic regression showed that only age and having pets had a significant association with intestinal parasites in this cohort. Conclusions: Our results confirmed that the evaluated population is at high risk of intestinal parasite infection, which highlights the need for routine screening of gastrointestinal parasites to provide prompt treatment and reduce possible complications.


Resumen | Introducción. La infección por HIV y las coinfecciones con otros agentes infecciosos, incluidos los parásitos intestinales, son motivo de especial preocupación en países en desarrollo como Colombia. Objetivo. Hacer un estudio transversal en pacientes que asisten a un programa de atención de HIV en el departamento de Antioquia, dado que los estudios de prevalencia de parásitos intestinales en la población con HIV son escasos en el país. Materiales y métodos. Se evaluaron 192 muestras de materia fecal mediante examen coprológico directo y por concentración, tinción de Ziehl-Neelsen modificada, y aislamiento en agar. Se hicieron análisis univariados y de correlación, para explorar la asociación entre las características sociodemográficas y clínicas, y los datos parasitológicos. Resultados. La prevalencia global de parásitos intestinales en pacientes positivos para VIH fue del 29.2 % (56/192; IC95% 22.8-35.6 %), siendo Entamoeba histolytica/dispar/moshkosvkii, con 13.0 % (25/192; IC95% 8.2-17.8 %), y Blastocystis, con 12.0 % (23/192; IC95% 7.4-16.6 %), los mas frecuentes. Los parásitos oportunistas Cryptosporidium spp. Y Cystoisospora belli fueron menos prevalentes, cada uno con 0.5 % (1/192; IC95% 0.1-1.5 %) de muestras positivas. También, se detectaron protozoos comensales, con una prevalencia del 18,8 % (36/192; IC95% 13,3-24,3 %). La mayoría de los individuos tenía una carga viral controlada y un recuento de linfocitos T CD4 superior a 200 células/μl. Un pequeño porcentaje (9,3 %) presentó diarrea. La edad y el tener mascotas mostraron una asociación significativa con la presencia de parásitos intestinales. Conclusión. Se confirmó que la población evaluada tiene un alto riesgo de infección por parásitos intestinales, lo que resalta la necesidad de un protocolo de diagnóstico para el cribado de dichos agentes, con el fin de brindar un tratamiento rápido y reducir las posibles complicaciones.

Abstract | Introduction: Strongyloides venezuelensis is a nematode whose natural host is rats. It is used as a model for the investigation of human strongyloidiasis caused by S. stercoralis. The latter is a neglected tropical disease in Ecuador where there are no specific plans to mitigate this parasitic illness. Objective: To evaluate the stages of S. venezuelensis in an experimental life cycle using Wistar rats. Materials and methods: Male Wistar rats were used to replicate the natural biological cycle of S. venezuelensis and describe its morphometric characteristics, as well as its parasitic development. Furthermore, the production of eggs per gram of feces was quantified using two diagnostic techniques and assessment of parasite load: Kato-Katz and qPCR. Results: Viable larval stages (Lr L2, L3) could be obtained up to 96 hours through fecal culture. Parthenogenetic females were established in the duodenum on the fifth day postinfection. Fertile eggs were observed in the intestinal tissue and fresh feces where the production peak occurred on the 8th. day post-infection. Unlike Kato-Katz, qPCR detected parasitic DNA on days not typically reported. Conclusions: The larval migration of S. venezuelensis within the murine host in an experimental environment was equivalent to that described in its natural biological cycle. The Kato-Katz quantitative technique showed to be quick and low-cost, but the qPCR had greater diagnostic precision. This experimental life cycle can be used as a tool for the study of strongyloidiasis or other similar nematodiasis.


Resumen | Introducción. Strongyloides venezuelensis es un nematodo cuyo huésped natural son las ratas. Se utiliza como modelo para la investigación de la estrongiloidiasis humana producida por S. stercoralis. Esta última es una enfermedad tropical desatendida que afecta al Ecuador, donde no existen planes específicos para mitigar esta parasitosis. Objetivo. Evaluar experimentalmente los estadios del ciclo de vida de S. venezuelensis utilizando ratas Wistar. Materiales y métodos. Se emplearon ratas Wistar macho para replicar el ciclo biológico natural de S. venezuelensis y describir sus características morfométricas y su desarrollo parasitario. Además, se cuantificó la producción de huevos por gramo de heces mediante dos técnicas de diagnóstico y valoración de carga parasitaria: Kato-Katz y qPCR. Resultados. Se obtuvieron estadios larvarios viables (L1, L2, L3) hasta las 96 horas del cultivo fecal. En el duodeno se establecieron hembras partenogenéticas a partir del quinto día de la infección. Se observaron huevos fértiles en el tejido intestinal inspeccionado y en las heces frescas, en las que el pico de producción ocurrió al octavo día de la infección. A diferencia del método Kato-Katz, la qPCR detectó ADN parasitario en días que usualmente no se reportan. Conclusiones. La migración larvaria de S. venezuelensis dentro del ratón en un ambiente experimental fue equivalente al descrito en un ciclo biológico natural. El método cuantitativo de Kato-Katz dio resultados inmediatos a más bajo costo, pero la qPCR tuvo mayor precisión diagnóstica. Este ciclo de vida experimental puede usarse como una herramienta para el estudio de la estrongiloidiasis u otras nematodiasis similares.

Introdução: as parasitoses intestinais constituem-se um importante problema de saúde pública mundial. Estas infecções são mais prevalentes em regiões tropicais impactando na morbimortalidade e aumento nos custos para o sistema de saúde. Objetivo: avaliar a prevalência das enteroparasitoses e sua associação com as condições socioeconômicas, sanitárias, ambientais e hábitos de vida em uma comunidade costeira do Nordeste brasileiro. Metodologia: estudo epidemiológico, descritivo e transversal, realizado de modo não probabilístico entre março a junho de 2017, com 105 moradores da Ilha de Boipeba, localizada no Sul da Bahia. O exame parasitológico de fezes foi realizado pelos métodos de sedimentação espontânea, Baermann-Moraes e FAUST. Um questionário foi aplicado para avaliar o perfil sociodemográfico da população. Resultados: do total de indivíduos avaliados, 52,4% eram do sexo feminino e 57,1% tinham entre 15 a 59 anos. Todos os indivíduos possuíam água encanada, porém não tratada, enquanto 91,4% referiu ingerir vegetais crus e 45,7% não higienizavam as mãos antes as refeições. Sintomas gastrointestinais foram relatados em 82,8% dos indivíduos e infecções enteroparasitárias foram diagnosticadas em 69,6%. Os parasitos mais frequentemente encontrados foram Ancilostomídeo (18,1%) e Entamoeba coli (43,8%). O principal fator de risco potencial para contrair a infecção por ancilostomídeos foi a não existência de poço artesiano na residência (RP=4,35), enquanto para Trichuris trichiura foi não dispor de pia no banheiro (RP=3,82). Conclusão: a comunidade analisada apresentou elevada prevalência de enteroparasitoses. Os hábitos precários de higiene e de acesso à água tratada, associados às condições ambientais e climáticas do local, podem ter contribuído para a elevada transmissão de geohelmintos observada.


Introduction: Intestinal parasitic infections are an important public health problem worldwide. They are more prevalent in tropical regions impacting in morbidity and mortality and costs for the health system. Objective: To evaluate the prevalence of enteroparasitosis and its association with socioeconomic, sanitary and environmental conditions and lifestyle habits in a coastal community in the Northeast Brazil. Methodology: This is an epidemiological, descriptive and cross-sectional study, carried out in a non-probabilistic manner between March and June 2017, with 105 residents of Boipeba Island in the Southern Bahia. The parasitological examination of feces was carried out by the methods of spontaneous sedimentation, Baermann-Moraes and FAUST. A questionnaire was applied to assess the sociodemographic profile of the population. Results: Of the total of individuals evaluated, 52.4% were female and 57.1% were between 15 and 59 years old. All individuals had piped, but untreated water, while 91.4% reported eating raw vegetables and 45.7% did not wash their hands before meals. Gastrointestinal symptoms were observed in 82.8% individuals and 69.6% were diagnosed with enteroparasitic infections. The most frequent parasites were the hookworm (18.1%) and Entamoeba coli (43.8%). The main potential risk factor for hookworm infection was the non-existence of an artesian well in the residence (PR=4.35) and Trichuris trichiura it was not having a sink in the toilet (PR=3.82). Conclusion: The analyzed community has a high prevalence of enteroparasitosis, in addition to environmental and climatic conditions that contribute to the transmission of these infections.

Resumen: Introducción: en el paciente crítico el intestino es protagonista de la respuesta al estrés. Los síntomas de insuficiencia intestinal en las unidades de cuidados intensivos no son específicos y no se incluyen en escalas de gravedad comúnmente usadas, sin embargo, el 62% de los pacientes críticos presenta síntomas gastrointestinales al menos una vez al día. Demostrándose la relación existente entre la aparición de problemas gastrointestinales y la mortalidad en pacientes críticos. Objetivo: proporcionar el conocimiento teórico necesario sobre el fallo intestinal como entidad exclusiva, su reconocimiento en el paciente grave, clasificación y enfoque multidisciplinario del tratamiento. Métodos: se realizó una revisión cualitativa y sistemática en los idiomas inglés y español de la literatura publicada y actualizada hasta febrero del 2020. Conclusiones: las alteraciones gastrointestinales son frecuentes en el paciente crítico y un determinante en su mortalidad. Reconocer el fallo y disfunción intestinal permite optimizar el tratamiento, reducir la mortalidad e incidencia de complicaciones, las unidades dedicadas con un equipo de experiencia en el manejo del fallo intestinal agudo constituyen un aspecto clave en este sentido.


Summary: Introduction: in critically ill patients, intestine function expresses the response to stress. Symptoms of intestinal failure in intensive care units are not specific and are not included in commonly used severity scales. However, 62% of critically ill patients present gastrointestinal symptoms at least once a day, demonstrating the relationship between the appearance of gastrointestinal problems and mortality in critical patients. Objective: to provide the necessary theoretical knowledge about intestinal failure as an exclusive entity and how to recognize it in critically ill patients, classification and a multidisciplinary approach to treatment of this condition. Methods: a qualitative and systematic review was carried out of the literature published and updated until February 2020 both in English and in Spanish. Conclusions: gastrointestinal disorders are frequent in critically ill patients and constitute a determining factor in their mortality. Recognizing intestinal failure and dysfunction allows the optimization of treatment, reduces mortality and the incidence of complications. The creation of dedicated units with experienced staff in the management of acute intestinal failure is a key aspect in this regard.


Resumo: Introdução: em pacientes críticos, o intestino é o protagonista da resposta ao estresse. Os sintomas de insuficiência intestinal em unidades de terapia intensiva não são específicos e não estão incluídos nas escalas de gravidade comumente usadas; no entanto, 62% dos pacientes criticamente enfermos apresentam sintomas gastrointestinais pelo menos uma vez ao dia, demonstrando a relação entre o aparecimento de problemas gastrointestinais e mortalidade em pacientes críticos. Objetivo: proporcionar o conhecimento teórico necessário sobre a insuficiência intestinal como entidade exclusiva, seu reconhecimento em pacientes críticos, classificação e abordagem multidisciplinar do tratamento. Métodos: foi realizada uma revisão qualitativa e sistemática nos idiomas inglês e espanhol da literatura publicada e atualizada até fevereiro de 2020. Conclusões: as alterações gastrointestinais são frequentes em pacientes críticos e determinam sua mortalidade. O reconhecimento da insuficiência e disfunção intestinal permite otimizar o tratamento, reduzindo a mortalidade e a incidência de complicações; contar com unidades dedicadas e com equipe experiente no manejo da insuficiência intestinal aguda são um aspecto fundamental nesse sentido.

Objective: We aimed to evaluate the prevalence of intestinal parasitic in the soil of three beaches of Mosqueiro Island, located in the State of Pará, Brazil, as well as to compare the frequency of helminths and protozoa, pathogenic and non-pathogenic parasites according to the beaches analyzed. Methods: This is a cross-sectional analytical study conducted during August and September 2019, in which 155 soil samples were analyzed by Hoffman's method. Results: The results showed that 16.1% of samples were contaminated from 61.3% of collection points. Murubira beach and Farol beach presented the highest prevalence of parasites, however, there was no significant difference between beaches. Also, it was observed a predominance of protozoa (63%) and non-pathogenic parasites (55.6%) in analyzed samples, but there was no statistically significant difference according to the investigated location. Endolimax nana 25.9% (7/27) and hookworms 18.5% (5/27) were the most detected parasites on the beaches. Conclusion: Thus, this study showed parasitic contamination on the beaches from Mosqueiro Island, which may be associated with a lack of sanitation infrastructure and personal hygiene in these places. Therefore, these results reinforce the need to adopt educational and preventive measures to reduce parasitic agents.


Objetivo: Avaliar a prevalência de parasitos intestinais no solo de três praias da Ilha de Mosqueiro, localizada no estado do Pará, Brasil, além de comparar a frequência de helmintos e protozoários, parasitos patogênicos e não patogênicos de acordo com as praias analisadas. Metdodos: Este é um estudo transversal, analítico, realizado durante agosto e setembro de 2019, no qual 155 amostras do solo foram analisadas pelo método de Hoffman. Resultados: Os resultados revelaram que 16,1% das amostras estavam contaminadas em 61,3% dos pontos de coleta. As praias do Murubira e do Farol apresentaram a maior prevalência de parasitos, porém não houve diferença significativa entre os diferentes locais de coleta. Além disso, foi observada a predominância de protozoários (63%) e parasitas não patogênicos (55.6%) nas amostras analisadas, mas não houve diferença estatística significativa entre os locais investigados. Endolimax nana 25,9% (7/27) e Ancilostomídeos 18,5% (5/27) foram os parasitas mais detectados nas praias. Conclusao: Desse modo, este estudo mostrou contaminação parasitária nas praias da Ilha de Mosqueiro, o que pode estar associada à falta de infraestrutura de saneamento e higiene pessoal nesses locais. Além disso, esses resultados reforçam a necessidade de se adotar medidas educacionais e preventivas para a redução desses agentes parasitários.

Schisandra Chinensis Fructus (SCF) is the fruit of Schisandra chinensis (Turcz.) Baill., a perennial vine. It was first recorded in Shen Nong's herbal classic and has a long application history. Studies have shown that SCF has anti-inflammatory, protective liver, antioxidant, antibacterial and other pharmacological effects. Ancient prescriptions are commonly used in the treatment of chronic diarrhea and other intestinal diseases and diabetes. Modern clinical phar?macology features of SCF polysaccharide (SCFP) in diabetes, liver diseases, enteritis and other aspects have achieved excellent results. Gut is an important digestive organ of human body, but intestinal diseases are varied, including Crohn's disease, ulcerative colitis, intestinal flora imbalance, etc.. It is a chronic and non-specific inflammatory disease. The disease is persisted for a long time and the incidence rate is expected to rise. Most of the symptoms are recurrent diarrhea, bloody stool and abdominal pain. It is considered by the World Health Organization as a refractory disease. At present, there is little possibility of complete cure, which is closely related to complex environmental factors, eating hab?its and heredity. In recent years, clinical studies have found that SCFP has a variety of pharmacological effects on intes?tinal protection.①Reduce inflammatory factors:intestinal mucositis is a common adverse reaction in patients with chemo?therapy. The development of mucositis is related to pro-inflammatory factors such as tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), IL-1β, Interferon-γ(IFN-γ). SCFP can significantly reduce IL-6 TNF-α, IL-1β, and IL-8, as well as the accumulation of T cells in the process of resisting apoptosis, reduce the inflammatory reaction and protect the dam?age to villi and crypts, improve the symptoms of small intestinal mucositis caused by weight loss and diarrhea. ② Pro?mote immunoglobulin A secretion: intestinal mucosal immunity is the first line of defense of the body's immune system. Its main antibody is secretory immunoglobulin A, which can destroy and phagocytize microorganisms, bacteria and viruses. SCFP can improve intestinal immunity by increasing the number and activity of T lymphocytes, promoting the secre?tion of secretory immunoglobulin A, and affecting the activity of a variety of cytokines. ③ Regulation of intestinal flora:the flora in the intestine has the functions of auxiliary nutrient absorption, biological antagonism and immune regulation, and can form a natural barrier for the host's intestine. When the human intestinal flora is disordered, probiotics will be greatly reduced, harmful bacteria will proliferate and destroy the intestinal environment. Under these conditions, the intake of SCFP significantly increased the number of beneficial bacteria such as bifidobacteria and lactobacillus, and sig?nificantly decreased the number of conditional pathogens such as enterococcus and escherichia coli, indicating that SCFP can indeed regulate the intestinal disorder caused by lincomycin hydrochloride to a certain extent. This may be because beneficial bacteria in the intestine metabolize polysaccharides produce short chain fatty acids such as lactic acid and acetic acid, which reduces the pH value in the intestine and inhibits the growth of enterococcus and Escherichia coli. In conclusion, SCFP can treat and protect intestinal diseases to a certain extent, which provides a favorable basis for the treatment of intestinal diseases.

Resumen La colitis ulcerativa es una enfermedad intestinal inflamatoria crónica, de etiología idiopática y fisiopatología multifactorial, caracterizada por la inflamación de la mucosa rectal, con extensión proximal, continua y simétrica a través del colon. Su diagnóstico se establece a partir de los hallazgos clínicos, endoscópicos e histológicos. La meta terapéutica es lograr la remisión clínica y endoscópica, reducir el riesgo de colectomía, cáncer colorrectal y mejorar la calidad de vida de los pacientes. Se realizó una búsqueda de artículos científicos y se exponen los aspectos más relevantes y actualizados sobre la enfermedad; con el objetivo de proporcionar elementos para su abordaje diagnóstico y terapéutico.


Abstract Ulcerative colitis is a chronic inflammatory bowel disease of idiopathic etiology and multifactorial pathophysiology. It is characterized by inflammation of the rectal mucosa with proximal, continuous and symmetrical extension through the colon. Its diagnosis is established based on clinical, endoscopic and histological findings. The therapeutic goal is to achieve clinical and endoscopic remission, reduce the risk of colectomy, colorectal cancer and improve quality of life of patients. A search for scientific articles was carried out, achieving this narrative review of the literature where the most relevant and updated aspects of the disease are exposed; with the objective of providing elements for the diagnostic and therapeutic approach of the disease.
